Form to be completed for the following children only; Judah, Lucas, Preston, John, Luan, Ollie, Louie, Anthony, Troy, Angus, Naz.

(Completing this form is considered consent for your child to attend this event. DO NOT complete this form if you do not want your child to participate).

WHEN:  Tuesday 28 April from 9.00am-2.00pm. Meeting at Taumata School staff room at 8.15am and returning to Taumata School in the afternoon at about 2.30pm.

WHERE:  Omokoroa No 1 School on Plummers Point Road (NOT IN OMOKOROA!)

COST:  No cost for this event.

PARENT HELP & TRANSPORT: We have 11 participants and 11 seats in the van so we DO NOT need transport to and from the event. Parents are still welcome to come to the event and spectate for some or all of the day.
